  • www.medqic.org: The CMS clearinghouse site MedQIC is an Internet website featuring a diverse collection of clinical intervention toll examples and other materials that support quality improvement projects (e.g., literature support for quality indicators, bibliographies, analytic flowcharts). Process-of-care flowcharts developed by the Quality Improvement Organization Support Centers (QIOSCs) for each clinical topic guide the user through quality improvement steps, barriers, change concepts, and suggested interventions and tools. Users of this site are QIOs and providers, including acute care hospitals, nursing homes, home health agencies and physician offices.
  • www.ihi.org: The Institute for Healthcare Improvement (IHI) is a not-for-profit organization driving the improvement of health by advancing the quality and value of health care.
  • www.improvingchroniccare.org: Improving Chronic Illness Care (ICIC) is a national program of The Robert Wood Johnson Foundation, based at the MacColl Institute for Healthcare Innovation, within Group Health Cooperative of Seattle. The program seeks to improve the care of the chronically ill through Improvement Collaboratives, the Targeted Research Grants Program, and a dissemination program.
  • www.healthdisparities.net: The Health Disparities Collaboratives strive to achieve excellence in practice through the following goals: generate and document improved health outcomes for underserved populations; transform clinical practice through models of care, improvement and learning; develop infrastructure, expertise and multi-disciplinary leadership to support and drive improved health status; and build strategic partnerships.
